新型冠状病毒肺炎员工症状证明

As on-site work restrictions are lifted by Governor Inslee, a daily 新型冠状病毒肺炎 symptom form will be required of employees returning to their on-site work location to ensure the safety and public health of the WVC campus and community. 表格是必需的 when individuals come to their work-site for any reason and for any length of time.

Whether individuals are working on- or off-site, all employees must:

If an employee directly informs you they are experiencing 新型冠状病毒肺炎 symptoms or do not complete the form because they are experiencing symptoms, instruct them to:

If you receive information that one of your employees is exhibiting 新型冠状病毒肺炎 symptoms or you observe an employee exhibiting 新型冠状病毒肺炎 symptoms, discuss what was observed, and if the employee acknowledges having the symptoms, instruct the employee to go home and contact their healthcare provider, and contact human resources (Tim标记 tmarker@cnhj88.com 或者里根·贝拉米 rbellamy@cnhj88.com

If the employee provides a rationale for the symptoms, such as seasonal allergies, determine whether the response is reasonable in relation to what has been observed. 接触人类 resources (Tim标记 或者里根·贝拉米) if you need assistance making the determination 这个人是否应该回家/在家工作.
